Things to do for the cacher who has done it all
Teach geocaching
Teach GSAK
Learn how to or teach how to solve puzzles
Write a book
Double up on Achievements (for example, if you have done the Fizzy Challenge, do it again)
Find more caches on particular days (for example, I have 3 days where the total of caches found is less than 100. For some reason the 18 of the month is the worst day for me to cache, and right now that day only has 47)
Find more of a particular kind of cache
Make creative cache containers and hides
Conduct Podcacher Podcast Field Interviews

A few ideas for the cacher that has accomplished "everything" built into the geocaching.com website:
There are many mystery caches around based on the fizzy challenge typically called "Well Rounded Cacher" or some other name.
Most states have the "County Challenge" to accomplish.
And as Electric Water Boy suggested, working on solving puzzle caches can be a great way to pass the time and give you something new to chase after. Better yet, create a series of your own local challenging puzzle caches.

Identify your own unique goal, achieve it, and challenge others to do likewise.

Scan those to find a new challenge goal and motivation!
Finally, since you have cached out your area, hide a spawn geocache that contains ready made caches that other finders can take and hide. This will give you some new caches to hunt when placed and approved.
